• Name and phone number of purchasing agent
• Name and phone number of end user
• Catalog number and description of product
• Quantity and size of product

To confirm your telephone order in writing, mark your confirmation clearly with "Confirmation — Do Not Duplicate".

If your institution is a regular customer, or if you order online, written confirmation of telephone orders is generally not necessary.
